Change of business name and address

Hello,
I am the owner of PW Chin Dental Sugery.
I would appreciate if you could add a note that P W CHIN Dental Surgery is now KLINIK PERGIGIAN MAWAR DENTAL and the new address is
1-3 Pusat Kreatif Kanak Kanak Tuanku Bainun,
48 Jalan Tun Mohd Fuad,
Taman Tun Dr Ismail
60000 Kuala Lumpur

Also please remove Dato Dr C T Chin as the second doctor in charge. Thanking you,
