I intend to come to your clinic in the week beginning 15. October, I´m arriving in KL on 16. October, coming from Germany.
in July 2011 I have already done a treatment with one of your colleagues, Dr. Yap in PJ. He fitted me 2 braces/bridges for the lower + upper jaws. Meanwhile I lost 2 teeth on the upper jaw and therefore now need a new piece. It should also be removable as-far-as-possible.

I was referred to you by my friend Mr. Lee Jaik Chin/Ms. Jessie Lee who are your patients. 

(I left Malaysia end 1969 and have settled in Germany since then.)
